Registration Information

 

Please contact the Counseling Office (217-222-4334) to register your child.

SCHOOL FEES:    The base registration fee is $80.00, which includes books and lock/locker.  An additional fee is charged for some classes. Fees may be paid in the Main Office.  Please wait until you see your child’s schedule to pay registration fees, as he/she may not be enrolled in the courses which charge an additional fee.  Make checks payable to Quincy Junior High School.

PHYSICAL EXAM AND IMMUNIZATION REQUIREMENTS FOR INCOMING 9TH GRADERS:

Immunizations may be obtained from your child’s physician or the Adams County Health Department.  The Adams County Health Department is located at 333 North 6th, and their phone number is 222-8440. The Health Department hours are Mondays 1:00-6:00 PM., and Wednesdays 1:00-4:00 PM. A physical examination and the Td (tetanus) immunization are required for incoming 9th graders.  If your 9th grader has not had a tetanus booster in the last ten years, he/she is due to receive one at this time.  Health forms may be obtained from your child’s doctor or from the Main Office at Junior High.  

 

AUTHORIZATION AND PERMISSION FOR ADMINISTRATION OF MEDICATION:  Whenever possible, medication should be given to students at home.  However, if students must take medication at school, an Authorization for Administration of Medication form must be completed in detail prior to prescription or non-prescription medication being administered. The complete guidelines are printed in the student handbook. 

 

BREAKFAST/LUNCH PROGRAM:  Student lunches will be $2.25; breakfast will be $1.25.  To receive a free or reduced breakfast or lunch, please complete an application for free and reduced-price meals.  Please list names and student i.d. numbers of all students attending Junior High on the same form.  Your child may eat lunch for free until your verification is returned to you.  Please remember that “free lunch verification” means no charge on book fees, and reduced lunch verification entitles students to half fees.  If your child received free/reduced lunch last year, you must fill out a new form for this year.   

 

Please note:  Families who receive food stamps or TANF benefits will receive a letter with an eligibility certificate for school meals and milk from the state.  If this applies to you, you do not have to complete the lunch application.  Instead, bring the eligibility certificate to the school with your registration forms, or as soon as you receive it in the mail. 

 

To pay for your child’s lunches, please place your money or check in an envelope.  Envelopes are also available in Main Office and cafeteria.  On the outside of the envelope, please write your child’s name, student i.d. number, and amount of money enclosed.  Deposits need to be taken to the Main Office.  You may also pay for meals online - instructions are available in the Main Office.   

   

ALL KIDS – OPTIONAL HEALTH CARE COVERAGE:  ALL KIDS is a state program that offers health care coverage to children, pregnant women and their babies or helps in paying premiums of employer or private health insurance plans.  Children, through age 18 and who meet income requirements, are eligible.  Income requirements vary by AllKids Plan and are based on family size.  Services are available at no cost or at low cost.  If you are interested, please call their toll-free number, 1-866-255-5437, to request an application, or visit the All Kids web site:  www.allkids.com

 

PTA:  The PTA plays an important role at Junior High.  Donations from the PTA fund a variety of school activities and projects.  Membership is $6.00 per person or $10.00 per family.  PLEASE JOIN!   

 

YEARBOOKS:   Order forms will be available at registration and in the Main Office. Cost is $30.00.
